How Gateway's Coordinated Entry Works

Access

The Gateway Center is the point of engagement with the coordinated entry/crisis response system. Persons experiencing homelessness will connect with a housing assessor.

Assessment

Client's needs are assessed by completing the VI-SPDAT assessment. Each household receives a vulnerability score that is used in determining prioritization for PSH.

Prioritization

Once the client’s information has been submitted to the housing queue and determined to be chronically homeless, they will be matched with a housing navigator.

Referral

Once the best housing intervention has been identified, a referral will be made to an agency that has vacancies. If both parties accept, paperwork is collected, leases are signed.
